Case Manager Home Care

Job Description:
Miwa Home Care is seeking a compassionate and organized Case Manager to oversee client services and ensure quality care delivery. You will coordinate with caregivers, clients, and family members to develop and implement care plans that align with our commitment to excellence.

Responsibilities:

• Develop individualized service plans in compliance with IDOA guidelines
• Conduct initial and follow-up in-home assessments
• Coordinate care between caregivers, clients, and families
• Monitor care delivery and client satisfaction
• Maintain accurate client documentation and records
• Ensure compliance with agency protocols and state regulations
• Communicate any changes in condition or care to appropriate parties

Qualifications:

• Bachelor’s degree in Social Work, Nursing, or related field (or equivalent experience)
• Minimum 1 year of home care or case management experience
• Strong knowledge of Illinois Aging Waiver Program is a plus
• Excellent organizational and communication skills
• Proficient in MS Office and electronic record-keeping
