Social Worker – Home Care

Job Description:
We are seeking a Social Worker to support our clients and their families with compassionate guidance and community resource access. The role focuses on psychosocial assessments, care coordination, and advocacy for elderly clients under the Aging Waiver Program.

Responsibilities:

• Conduct comprehensive psychosocial assessments
• Assist clients and families with accessing community services and benefits
• Provide emotional support and crisis intervention as needed
• Collaborate with case managers and caregivers on service plans
• Document all social work activities and maintain client confidentiality
• Ensure compliance with IDOA and IDPH regulations

Qualifications:

• Bachelor’s or Master’s in Social Work (BSW or MSW) required
• Licensed Social Worker (LSW) in Illinois preferred
• Previous experience with elderly populations or home care is a plus
• Strong interpersonal and cultural sensitivity skills
• Familiarity with Medicaid waiver services a bonus
