Job Description

Key Responsibilities

  • Follow Care Plans: Implement the participant’s person-centered plan of care as directed.
  • Complete Assigned Duties: Carry out tasks as assigned by the supervisor, ensuring quality service delivery.
  • Monitor and Report: Observe participant functioning and report any changes to the homecare supervisor.
  • Document Activities: Maintain accurate records of daily activities, observations, and hours of service provided.
  • Assist with Shopping: Provide necessary receipts and documentation for errands and essential shopping.
  • Participate in Training: Attend pre-service and ongoing in-service training sessions, including staff conferences.

 

Qualifications:

  • Education: High school diploma or GED; OR one year of relevant experience; OR progress towards GED with successful coursework.
  • Training: Completion of 24 hours of initial pre-service training, including agency orientation and competency evaluation.
  • Ongoing Education: Minimum of 12 hours of in-service training annually, including two hours focused on dementia care.
  • Skills: Strong communication skills, ability to work with diverse populations, and understanding of basic hygiene and safety practices.

 

Additional Requirements:

  • Completion of mandatory dementia training within 30 days of employment and annually thereafter.
  • Ability to assist with personal care tasks and recognize changes in participant condition.
